Lost Claim

To submit a claim for lost shipments, please remember to include:

• A completed and signed claim form
• Wholesale cost invoice for lost item
• Copy of the shipping waybill/manifest
• All claims must be submitted within 9 months of the ship date

Damaged Claim

To submit a claim for damaged shipments, please remember to include:

• A completed and signed claim form
• Wholesale cost invoice for damaged item
• Copy of the shipping waybill/manifest
• Picture of the damaged item (or the damaged item may be returned to the Claims Dept with the submission)
• All claims must be submitted within 9 months of the ship date
